Organizations have struggled for years to find solutions to our healthcare challenges. A large opportunity exists to dramatically reduce chronic disease-related healthcare costs and improve quality of care and life for underserved, rural and hard to reach patients. However, many of the solutions developed over the past 20 years have yet to find the right formula for addressing delivery system challenges while bridging the largely uncoordinated relationships between patients, physicians and payers. Additionally, SJH achieved an 83% patient satisfaction rate.
