World Congress is pleased to present the Leadership Summit on Intersection of Home Care Monitoring and Consumer Engagement. Challenges such as geographic location, mobility, and time constraints often keep populations, specifically high risk populations that are homebound or live in rural areas, from accessing quality health care. New technologies such as telehealth and online care work to remove these barriers and provide patients with timely, quality, cost effective care. These technologies have proven to engage consumers by giving them greater control of their health care experience in a user friendly convenient format. This unique forum will provide strategies for senior executives from hospitals, health systems, and health plans to leverage online care and telehealth in order to improve quality of care and access to care, while also bending the cost curve.

Topics to be Addressed Include:

• Improving quality of care by enabling physicians to manage to needs of patients in real-time
• Engaging patients in their health care by increasing convenience, usability, and access
• Shifting towards an ACO model –  providing continuity of care across organizations
