UnitedHealthcare has introduced a disease specific health plan designed to help diabetics and pre-diabetics manage their conditions more
effectively while controlling costs. The plan rewards diabetic and
pre-diabetic individuals who routinely follow medically proven steps that improve their conditions (i.e. blood sugar checks, routine exams and preventive screenings) and that use wellness coaching.
- Learn the value of engaging individuals in a program designed with personalized, specific self-management steps that maximizes
opportunity for turning the disease around
- Examine 1 ½ years of data that demonstrate the program’s
effectiveness in improving compliance and member health outcomes
- Discuss future plans to expand program services and
benefits for diabetes as well as other conditions - asthma,
hypertension, muscular-skeletal disorders and maternity
