| As one of the largest transportation companies in the U.S., Schneider National wanted to implement a unique disease management program, within their consumer-driven plan, designed to manage a specific medical issue facing their truck driver population, Sleep Apnea, and have it covered as 100% preventative care. This presentation will cover how developing a unique, employee population specific disease management program can have a significant impact on reducing costs, hospitalization, and employee retention. |

| In this session, learn why the American Postal Workers Union Health Plan (APWUHP), a self insured, self administered health benefit plan operating under contract with the United States Government, Office of Personnel Management, as a participating plan under the Federal Employees Health Benefit Program (FEHBP), began offering their constituents a consumer-driven option and how their health plan developed individualized health messages to increase adherence to evidence based guidelines for certain preventative screenings. Individualized messaging campaigns on mammograms and cervical cancer screenings will be highlighted where the APWUHP realized a 62% increase in mammogram screenings and 68% increase in cervical cancer screening thereby improving employee health and decreasing potential medical spend. |
