CASE STUDY: Integrating Physicians, Employers and Health Plans to Form a Medical Home and Collaborate on Benefits and Products Offered
Adirondack Medical Home pilot which is a unique project with participation by six commercial health plans, including both for-profit and not for profit carriers; NYS Medicaid; NYSHIP, the NYS government employees plan; Medicare and approximately 100 primary care physicians who have organized into three “pods” to transform their practices into NCQA accredited level 2 or level 3 medical homes. T his project is especially interesting in that it is operating in the Adirondack Park, a 6.1 million acre, largely rural area with significant geographic and economic barriers to the provision of high quality primary care.
