Ms. Wendy Cofran has been Chief Information Officer for Century Health Systems, Inc., the parent company of Natick Visiting Nurse Association (Natick VNA) and Distinguished Care Options (DCO) for over two decades. In this executive management role, Cofran has developed, and managed with unparalleled success cutting edge technology that is focused on enhancing quality, improving safety, reducing costs and increasing efficiency across the healthcare system.

As the national conversation turns to using technology to solve the challenges of improving health care outcomes, providing a 360 degree patient experience and driving down escalating costs Ms. Cofran's innovative, collaborative and “out-of-the-box” approach has set Century Health Systems and its affiliates apart from the crowd. In 2012 she developed and launched a Referral Management application wholly built on the Salesforce platform. This application increases clinical staff efficiency, provides more timely clinical documentation, and leads to improved patient outcomes and experience with the homecare agency. As the federal government moves to Value Based Payment for all health care providers, these positive results are also helping decrease the overall cost of providing care. Jess Dalton is a seasoned acute and post-acute healthcare executive with expertise in cross-continuum operations, service line management, physician relations, business development, and financial advantage. As a strategic and operational leader, Jess has an impressive track-record in both acute and post-acute levels of care - with expertise in strategic ACO development and alignment, Episode & Alternative Payment Model development and implementation, translational care continuum/pathway development, episode of care efficiencies, safe and seamless patient transitions, value-based purchasing, and population health management. Dr. Kathryn Fiddler is presently the Executive Director for Population Health Services at Peninsula Regional Medical Center. She has over 20 years of experience in clinical and military leadership, including work with physician practices, payers, accountable care organizations and managed care. She presently leads Population Health efforts at PRMC focusing on care management within PRMC, the community and the Peninsula Regional Clinically Integrated Network. She also leads the Community Initiatives teams as well as the Peninsula Regional Weight Loss and Wellness Center. Dr. Fiddler received her Doctoral degree from Salisbury University in Salisbury, MD and is retired from the United States Air Force Reserve.

Steve is Director of Kindred Innovations, focused on developing care management solutions aimed at resolving the gaps in the care delivery system, from traditional provider health systems to post-acute care systems across ACOs and other risk bearing entities. Before joining Kindred Innovations, Steve served as a Director of Kindred Strategy where he led the development of enterprise strategy and the formation of strategic partnerships and customer relationships. Prior to 2015, Steve led integrated market operations in Boston across Kindred’s post-acute care continuum of services, designing and implementing Kindred’s first Care Management program, which successfully reduced post-acute length of stay and readmissions while improving patient and family experience. During this time in the Boston Integrated Market, Steve also served as an Assistant Administrator of one of Kindred’s long-term acute care (LTAC) hospitals. Steve joined Kindred in 2010. He earned his undergraduate degree in Biology from the University of Pennsylvania and MBA from Boston University.

Mark Green is currently Ochsner Health Systems Assistant Vise President of their Post-acute network, and Transitional Management division, including the Ochsner Care Coordination Center (C3). Mark is seen as industry expert in new and emerging Care Coordination efforts, and in developing leading edge transitional care models in our changing healthcare settings. In his previous roles Mark has held positions in the industry as a credentialed healthcare portfolio manager, and a practicing lean six sigma black belt.

As the Senior Director for Care Continuum at Mount Sinai Health System, Esther is responsible for the health system's post-acute care strategy and operations. She engages skilled nursing facilities, home care agencies, and hospices to participate in network development and performance improvement collaborations. Esther leads a team of nurses, who manages patient care in the post-acute care setting. This team focuses on quality, outcomes and utilization.

She has spent 10 years in post-acute care operations, where she was a Regional Director and an Administrator for a Certified and Licensed care agency. Prior to joining Mount Sinai Health System, she was a Director at ArchCare, the post-acute care continuum for the Arch Diocese of New York, which has 5 Skilled Nursing Facilities, a Certified Home Health Agency, Licensed Home Care Service Agency, a PACE, an MLTC, and a Medicare Advantage. Esther received her nursing degree and Masters of Science from NYU. She is LEAN Six Sigma Green Belt Certified.

Matt Murphy is currently President and CEO at Griswold Home Care, one of the nation's oldest homecare franchise brands. Griswold operates over 200 locations in 32 states. Prior to Griswold, Matt was CEO of FHS, a revenue cycle management business that services the senior care market.

Prior to FHS, Matt held sales and operations leadership positions with several business services companies. He was also an attorney for the University of Pennsylvania Health System, and he began his career as a Naval Officer.

Matt holds a B.S. from Rochester Institute of Technology, and a J.D. from the State University of New York at Buffalo.

Sneha is a Senior Health Program Planner in the Office of Policy and Planning at the San Francisco Department of Public Health. Her work focuses on citywide health planning, policy development, policy analysis, and research to improve access to health care for persons who are low-income, including seniors and persons with disabilities. Sneha received her Master in Public Health from Columbia University. Prior to working at the San Francisco Department of Public Health, Sneha worked for the NYC Department of Public Health, the American Geriatrics Society, and University of California San Francisco Division of Geriatrics. She received her undergraduate degree from University of California, Berkeley. In 2016, Sneha co-authored the report Framing San Francisco's Post-Acute Care Challenge which identities several local post-acute care challenges including San Francisco's declining skilled nursing bed supply and barriers to patient access.

Elaine M. Ryan is the Vice President of State Advocacy and Strategy Integration (SASI) in the Government Affairs Department of AARP. Elaine leads a team of legislative staff who work with our 53 AARP state offices to advance AARP’s state advocacy agenda with Governors and state legislators throughout the nation to enable individuals age 50+ and their families attain and maintain their financial and health security. Our priority legislative issues include supports for family caregivers; small business access to workplace savings; health care, and consumer issues.

Prior to joining AARP in 2007, Elaine served as Deputy Executive Director of the American Public Human Services Association—a bi-partisan membership organization representing the 50 state Governor’s cabinet officers of health and human services agencies and the National Association of State Medicaid Directors. Elaine also served as Chief of Staff and House Budget Staff Associate in the U.S. House of Representatives and she served on the senior staff of New York State Governor Mario Cuomo for nine years.

Through her career, Elaine has worked on a number of landmark task forces, federal and state laws including the nation’s first Task Force on AIDS, the creation of the Children’s Health Insurance Program, the Welfare Reform law, the Welfare to Work Program, the Ticket to Work Program, the creation of the first US office of Women’s Health, the Homeless Children Education program and the Affordable Care Act. Her state advocacy work has led to the enactment of the nation’s first Work and Save workplace savings programs; the Caregiver Advise, Record and Enable (CARE) Act which she co-authored and enacted in 31 states; and over 100 state laws to support family caregivers and their families.

Elaine earned her Master of Public Administration from the John F. Kennedy School of Government at Harvard University and her undergraduate degree in Economics from Fordham University. She is the recipient of the Council of State Governments Toll Fellowship, the Exceptional leadership award from the American Public Human Services Association. Elaine has been featured in The New York Times, the Washington Post, the National Journal, the Huffington Post and other newspapers and magazine. She has served as a member of numerous corporate, university and local government boards and commissions. She currently serves as a National Advisory Board member of Open Notes, a project of Harvard Medical School and Beth Israel Deaconess Medical Center, designed to empower patients to become more engaged in their own health care.

Susan C. Westgate, MBA, MSW, LCSW-C is a board approved clinical supervisor in the state of Maryland. She obtained her Masters in Social Work at the University of Maryland, School of Social Work and a Masters in Business Administration with a focus on Leadership and Management at the University of Baltimore. She is currently employed as a full time Clinical Instructor at the University of Maryland, School of Social Work. She also currently serves the Director of Community Care Coordination at LifeBridge Health. Prior to this current position she was instrumental in assisting with Quality and Post-Acute Initiatives in LifeBridge's Skilled Nursing Facility (SNF) Collaborative. Prior to her current work, she served as an Inpatient Social Worker at LifeBridge and developed expertise in critical care, oncology, hospice and palliative care, and trauma. Teaching is of central importance to Susan and she has taught and has forthcoming lectures on a variety of topics in professional development venues on subjects such as: healthcare forecasting, end of life care, supporting oncology patients and their families, industry and institutional challenges to hospital care, Emergency Department diversion programs, Population Health Management, and vicarious trauma. As a faculty member in the University of MD, School of Social Work’s graduate program she has received numerous teaching awards including the of the Exemplary Faculty Member of the Year Award along with the Dean’s Teaching Award for several consecutive semesters. She has been published in The New Social Worker, The Journal of Employee Assistance, Infusion Magazine, Provider Magazine, and Social Work Today.

Pierre L Yong, MD, MPH, MS, is the Director of Quality Measurement in the Quality Measurement and Value-Based Incentives Group/Center for Clinical Standards and Quality at the Centers for Medicare & Medicaid Services (CMS). At CMS, he oversees measure development and analyses for a variety of CMS quality reporting and value-based purchasing programs. Previously, Pierre was the Director of Health Care Quality and Outcomes in the Office of the Assistant Secretary of Planning Evaluation at the U.S. Department of Health and Human Services, where he oversaw projects on value based purchasing, quality, comparative effectiveness research, and data infrastructure for patient-centered outcomes research. Pierre also previously worked at the Institute of Medicine, where he focused on a portfolio of projects related to value and costs in health care. As a Robert Wood Johnson Clinical Scholar at the University of Pennsylvania, his research focused on quality of care. Pierre received his medical degree from Brown Medical School, his Masters in Public Health from Columbia University, and his Masters in Science in Health Policy Research at the University of Pennsylvania. He completed his Internal Medicine training at New York Presbyterian Hospital, and most recently worked as a hospitalist at the District of Columbia Veterans Affairs Medical Center.

Ruth is currently the Post-Acute Care Manager for the San Francisco Bay Area Dignity Health market. Ruth started her career as an emergency department nurse and previous to the current role worked as a Program Manager tasked with designing and implementing a transitions of care program concentrating on reducing avoidable readmissions. This past summer, Ruth co-authored a white paper titled “Partnerships in Transitions: Acute Care to Skilled Nursing Facility” about the transitions of care program. Ruth is a frequent speaker at transitional care conferences and webinars and provides consulting services on the topic of health care changes to hospitals, physician groups and health care affiliated technology companies. Ruth has a Bachelor of Science in Nursing and Master of Science in Nursing and Health Systems Leadership.
