Mike Adelberg has 25 years of progressive experience with Medicare, Medicaid and the Health Insurance Exchanges. Before joining Faegre Baker Daniels Consulting three years ago, Mike held several senior positions within the Centers for Medicare and Medicaid Services (CMS), including concurrently serving as the director of the Insurance Programs Group and the acting director of the Exchange Policy and Operations Group in the Center for Consumer Information and Insurance Oversight (CCIIO). Prior to that, Mike was the Director of Medicare Advantage Operations, where he supervised the annual cycle for review and award of Medicare Advantage contracts, developed CMS’s operational policy, and led the monitoring of Medicare Advantage contractors. His other senior roles at CMS included serving as the associate regional administrator for Medicare operations (Chicago Region) and the director of education and assistance programs. Mike gained private sector experience while serving as vice president of product development and government affairs for a multi-state health insurer. Mike also co-led a successful health policy and program evaluation practice for national consultancy. Mike speaks and publishes frequently on healthcare reform topics and has been quoted in the Washington Post, New York Times, Modern Healthcare, NPR, and other leading media. In his spare time, Mike is an author. He has authored three novels, a history book, several scholarly journal articles, and over sixty book reviews.

Dr. Aluisio joined Greensboro Orthopaedics in 1998 and is a board-certified, fellowship-trained, orthopaedic surgeon. He specializes in complex total hip and knee joint replacement surgery as well as provides treatment for degenerative conditions of the hip and knee joints. Dr. Aluisio is the most experienced adult reconstructive hip and knee surgeon in the Triad. In fact, since joining Greensboro Orthopaedics, he has performed the most hip and knee replacements in this area. He combines well-proven techniques with cutting-edge technology to provide the best possible care for his patients.

Dr. Aluisio has published numerous articles and book chapters, presented both nationally and internationally, and co-authored "Orthopaedics for the House Officer, 2nd Edition." He has also been an instructor for several hip and knee replacement courses. Dr. Aluisio is a former President of the North Carolina Orthopaedic Association and is currently on the Board of Councilors for the American Academy of Orthopaedic Surgeons. Through community involvement, Dr. Aluisio shows his commitment to both his patients and extended community. Recently, he has provided healthcare treatment educational information sessions for the aging population. As a member of the Greensboro Sports Council, Dr. Aluisio strives to provide hospitality, resources, and community support for local event participants. In the past, he has also served as a youth sports coach.

Providing patient-centered care is a priority for Dr. Aluisio. He prides himself in building positive relationships with his patients. He appreciates being able to serve his patients and enabling them to return to a comfortable and active lifestyle.

Richard J. Brockman is one of the founders, a director, and chair of Simpra Advantage, Inc., an Alabama statewide based Medicare Advantage Special Needs Plan, and its sister company, Alabama Select Network, LLC, which is developing a provider driven statewide Alabama Medicaid long term care management program in Alabama. He also is a founder, director and chair of SeniorSelect Partners, LLC, which is the holding company for these entities.

Richard is also lawyer and is counsel in the Birmingham Alabama office of Burr & Forman LLP, and is a member of the law firm’s Health Care Practice Group. His practice includes representing a variety of health care providers, from the largest health care systems to individual providers on an array of health care related legal issues, with a focus in the long term care sector. He also represents regional and national lenders and mortgage servicers in matters concerning seniors housing loans. In addition, he is a principal of PRN Capital, LLC and The Survey Companies, LLC, specialty companies providing underwriting and industry advice to commercial mortgage banks, agency lenders, and servicers of mortgage loans secured by senior housing projects. He also owns and operates seniors housing facilities in Alabama and Pennsylvania.

Over the years, Richard has been active in drafting and responding to national and state legislation affecting health care providers and consumers. In 2015, he was one of the principal authors of legislation creating an Alabama Medicaid long term care management program called the "Integrated Care Network. In addition, he has been a principal author of various state legislation or amendments to legislation such as The Alabama Natural Death Act, The Alabama Durable Power of Attorney Act, The Alabama Adult Protective Services Act, The Alabama Medical Liability Act, Provider Specific Taxes, Certificate of Need Laws, and various state health facility licensure laws.

He is a past chair of the Alabama Nursing Home Association (having served 4-terms as chair) and, serves on the legislatively created Alabama Statewide Advisory Council on Palliative Care and Quality of Life. In addition to serving on the Legal Committee, Quality Improvement Committee, and Reimbursement Committee of American Health Care Association, he is a member of the American Health Lawyers Association and a host of other legal and health related organizations. Richard is a frequent speaker both locally and nationally on a variety of health care matters.

Richard received his B.A. in accounting from Huntingdon College, his law degree from the Cumberland School of Law, Samford University, and his masters of law in taxation from New York University School of Law.

John Capasso is currently serving in the role of executive vice president of the Continuing Care Group at Trinity Health. Trinity Health continuing care services consists of home health, hospice, senior living, nursing homes and Programs for the All-inclusive Care for the Elderly. This array of services exists in multiple markets throughout the 20 states in which Trinity Health is located. Annual revenues for these services are approximately $1 billion per year. Mr. Capasso has more than 35 years of experience in health care and senior living services.

Prior to this role, Capasso served as the senior vice president of the Continuing Care Management Services Network at Catholic Health East (CHE). Capasso also served as the president and chief executive officer of St. Joseph of the Pines Health System beginning in 2001, a continuing care ministry providing a variety of housing and health care services to seniors in central North Carolina. Prior to joining CHE, Capasso served in a variety of executive roles at Asbury Services, Gaithersburg, MD, a provider of senior living services across multiple states. Prior to his various roles in continuing care, Mr. Capasso worked in positions in four acute care hospitals culminating in the role of assistant vice president, Professional Services, Holy Cross Hospital, Silver Spring, Maryland.

Capasso has a Master of Science in health services administration from George Washington University; a Bachelor of Arts in biology from Geneva College, Beaver Falls, PA.; and is a certified health care executive with the American College of Healthcare Executives. He is also a licensed nursing home administrator.

Chaffee was recognized as a 2017 Healthcare Hero by the publication Business West & Healthcare News, specifically earning the honor as the Health/Wellness Administrator of the year. Andrea Cichra is the Director of Population Health at The South Bend Clinic, a large physician-owned multi-specialty clinic located in South Bend, Indiana. As Director of Population Health, Mrs. Cichra is responsible for developing and implementing strategic initiatives and operational programs that bring value to population health management and the Clinic’s overarching strategy surrounding value-based care.

Mrs. Cichra is responsible for management and oversight of the Clinic’s Track 3 MSSP ACO as well as numerous other shared savings, pay for performance, ACO, and value-based payer contracts for Medicare Advantage, commercial, and managed Medicaid populations. Mrs. Cichra serves as the Clinic’s key contact with external stakeholders regarding population health platform and strategy, governmental advocacy, and policy. She directs network management, data operations and analysis, care management and transitions management departments, Hierarchal Condition Coding (HCC) performance, contract and quality management, and payer relations. She also designed and directly oversees the Clinic’s Post-Acute Network.

In her work with population health strategy, two strategic goals that Mrs. Cichra has been passionate about have been post-acute evaluation with network development and translating data into actionable, operational programs and tools to drive performance results and provider engagement while reducing provider burden and burnout. Working with her team and LEAN processes, Clinic-wide programming has been established or is currently being piloted to address quality of care improvements, reimbursement, cost of care management, provider burden, administrative burden, and clinical programming to meet patient needs.

In her healthcare career, Mrs. Cichra has gained valuable experience in diverse healthcare spaces to include skilled nursing, home health, assisted living, hospice, CCRCs, memory care facilities, private, public, and military hospitals, outpatient clinics, and ambulatory surgery centers. Licensed as a Health Facilities Administrator, Mrs. Cichra is proud to have served as Executive Director/CEO in the post-acute space and purposefully maintains her HFA licensure. She is also a Marine Corps veteran and an alumnus of Indiana Wesleyan University.

With offices in Washington, DC, and Boston, Massachusetts, NEHI conducts independent, objective research and thought leadership to accelerate these innovations and bring about changes within health care and public policy.

One of the nation's most respected health and health policy thought leaders and journalists, and a frequent speaker and commentator on television and radio, including NPR, Dentzer previously served as senior policy adviser to the Robert Wood Johnson Foundation. Prior to that, she was editor-in-chief of the journal Health Affairs, and from 1998 to 2008, she served as the on-air Health Correspondent for the PBS NewsHour.

Dentzer is an elected member of the National Academy of Medicine and the Council on Foreign Relations. She is a member of the Board of Directors of the International Rescue Committee, a leading humanitarian organization; a member of the board of directors of Research!America, which works to advance research to improve health; and a member of the board of directors of the Public Health Institute. She is a fellow of the National Academy of Social Insurance and the Hasting Center, an institution dedicated to bioethics and the public interest. She previously served as a public member of the American Board of Medical Specialties. She serves on multiple advisory boards, including those of the Duke-Margolis Center; the Philip R. Lee Institute for Health Policy Studies at the University of California-San Francisco; and the March of Dimes public policy advisory group.

A graduate of Dartmouth, Dentzer formerly chaired the Dartmouth Board of Trustees, and is a member of the Board of Advisors of Dartmouth Medical School. She and her husband, Charles Alston, have three grown children.

Emily Engel is Deputy Director for the Bureau of Social Determinants of within the Office of Health Insurance Programs (OHIP) at the New York State Department of Health (DOH).

Engel’s work is focused on creating better health outcomes for vulnerable Medicaid members through innovative policy and program approaches. She leads a team in implementing the Value Based Payment (VBP) requirements for Social Determinants of Health and Community Based Organizations. She also manages Governor Andrew Cuomo’s $880 million-dollar Medicaid Redesign Team Supportive Housing Initiative. Prior to the Department of Health, Engel worked with Section 8 housing sites to uphold safe and affordable housing. She also worked for the New York State Assembly, where she created custom communication plans, policy and media related statements for assembly members.

Emily Engel received an MBA from Northeastern University and a Bachelor of Science in Public Policy and Public Affairs from The Sage Colleges, where she was named valedictorian.

Trained as an internist at UCSF, Dr. Johnston is currently Medical Director at CareCentrix, a post-acute benefits management company. She started her career as a partner in a 50-physician multi-specialty group practice. She subsequently shifted to positions as medical director and chief medical officer at cardiovascular prevention programs ranging from academic to commercial to venture-capital backed startups.

Most recently, she co-founded a self-funded start-up, eMedBracelet, an electronic personal health record. She strongly believes that individuals (or their surrogates) must control their own medical records, accessible from wherever they may find themselves, and maintain their own disease prevention schedules while incorporating lifestyle strategies like diet and exercise that will optimize their health.

She has a passion for global health and serves on the board of Bridges to Community, a non-governmental organization working in Nicaragua and the Dominican Republic. Dr. Johnston chairs the group within BTC that is building a medical center for 18,000 individuals with minimal access to health care at present. The center will open in May, 2018.

Her goal was to make Juniper profitable for investors; and as a founder of the then nascent social corporate social entrepreneurship movement, she would adhere to the double bottom line: doing well by doing good.

Today, 30 years later, CEO Katzmann continues to actively lead Juniper, now with 21 properties in four states, more than 1550 employees, and $85 million in revenues. Juniper is ranked #12 by Crain’s New York on its annual Top 50 ranking of woman-owned companies for the New York tri-state area.

She directs a team of population health professionals in delivering value-based care models and forging closer relationships with referral partners, including Accountable Care Organizations (ACO). With a focus on developing disease solutions and programs, Kipple’s aim is to create outcomes that will maximize the patient’s clinical experience while managing costs to achieve superior outcomes.

Kipple has specifically worked in the home health care sector since 1997, with long-term stints held at Baptist Hospital Home Care, Gentiva Health Services, and Advanced Home Care. She has consistently challenged herself to integrate strong clinical programs using innovative industry technology. She has designed and implemented specialty programs that include cardiopulmonary, wound care, disease management, and has built strategic partnerships with a variety of health care organizations. Kipple has directed clinical and practice outcomes for 15 health care sites across three states, with oversight of 750 clinicians. In addition, she has had successful oversight of Model 3 bundle payment model in those 15 locations. Kipple is also a proud member of the 2018 Advisory Board for the Home Care Technology Association of America (HCTAA).

Rose Mary Madden-Baer is the Senior Vice President of Population Health and Clinical Support Services at the Visiting Nurse Service of New York (VNSNY) one of the largest not-for-profit home health agencies in the country. Prior to that role she held positions in Clinical Operations Strategy and Development of Behavioral Health, Assessment Services and Special Programs at VNSNY. Dr. Madden-Baer is also a Consultant Associate Professor at Duke University and an Assistant Clinical Professor at Yale University. Dr. Madden-Baer serves as Chair of the National Advisory Board for Duke University School of Nursing’s Executive Leadership Track DNP program. Prior to VNSNY she held positions as the Vice President for Community Programs at the St. Mary’s Healthcare System for Children and the Vice President for Quality Management and Clinical Services at Metropolitan Jewish Health System.

Dr. Madden-Baer earned her undergraduate degree from Long Island University and her graduate nursing degree at Hunter College. She holds a MHSA from St. Joseph’s College and a DNP from Duke University. She also received an Executive Leadership Certificate from Cornell University. She is a board certified clinical nurse specialist in community health and received post-graduate certificates from Duke University in Population Care Coordination (PCC) and in Population Health from the Jefferson College of Population Health. Dr. Madden-Baer was inducted as a Fellow into the American Academy of Nursing, and as a Fellow for the Hartford Institute for Geriatric Nursing. Dr. Madden-Baer has served on the CMS-Technical Expert Panels for Population Health Measures: Multisector Collaboration in 2016, in Home-Based Care Models in 2017 and Home Health Payment Models in 2018. In addition, Dr. Madden-Baer served on the Care Models Governance Committee for One City Health, the largest performing provider system in New York State’s Delivery System Reform Incentive Payment (DSRIP) program. In 2014, Dr. Madden-Baer was awarded the Innovative Leader of the year award by the Visiting Nurse Association of America (VNAA) and in 2014 the Duke University School of Nursing’s Distinguished Alumna award.

Dr. Madden-Baer has published and presented nationally on care model innovation “in home-visit based programs” including her oversight of VNSNY’s BPCI Model 3 bundle development and implementation since 2013 and VNSNY’s various value-based care models with health plans since 2015. In 2015, Dr. Madden-Baer participated as a panelist for “Exemplar of Models of Care in Approaches to Payment” for the National Academies of Health and Medicine (formerly the Institute of Medicine-IOM) National Research Council (NRC). Madden-Baer’s work was described in “The Future of Home Health Care: Workshop Summary”. (P.77-79.) Kim leads a team of experienced RN Care Managers, Health Coordinators, Licensed Masters Social Workers, and Coders. Together, they are responsible for managing the health of patient populations through care management programs that include complex disease management, gaps in care services, psychosocially complex case management and transitional care management – facilitating patient continuity of care during post hospital transitions.

As the head of Comprehensive Care Management, Ms. Morris oversees a vital element of population health. She directs care management strategy for outreaching 60,000+ high- and rising-risk BSWQA patients, drives accountability among BSWQA’s network of post-acute providers and guides other initiatives such as BSWQA’s HealthAccess navigation services and Clinical Documentation Improvement.

Glen Roebuck has served as a leader in senior health care and post acute care for over 30 years. His career has spanned a wide range of positions with growing responsibility. Mr. Roebuck has been a licensed Nursing Home Administrator in three states, and served as a regional director of senior care operations in numerous states throughout the Midwest and Mountain West. He has also served in strategic leadership roles, including operational guidance, business development and consultation, in environments ranging from single ownership models to large, statewide health care systems.

In his current role, Mr. Roebuck serves as Executive Director of Post Acute Care for Genesis Health System, based in the Quad Cities region of Iowa and Illinois. He has responsibility for operational performance of senior living facilities, community and inpatient hospice services, and a large home health care agency, as well as a new palliative care program. In addition, Mr. Roebuck has implemented an outreach program placing Genesis Nurse Practitioners within high volume post acute care sites, dramatically impacting quality metrics and readmissions. He also maintains regular communication with senior health care facilities throughout the Genesis service area, addressing health system throughput, interoperability of EMR systems, and opportunities for improved collaboration to achieve Triple Aim performance within the community. In addition, he is actively engaged in the oversight and post acute engagement of the Genesis ACO.

Through community partnerships and collaborations, Mr. Roebuck has developed the Genesis Senior Services partnership, a collaborative of Genesis and community providing access to trusted, proven providers committed to excellence and a shared vision to serve the Quad Cities senior community. Mr. Roebuck has published numerous professional articles on topics such as the successful leadership of skilled centers during challenging economic conditions, as well as preparation for health care reform and participation in Accountable Care Organizations. David A. Shore is a leading authority on innovation and managing change initiatives to gain competitive advantage. Throughout his career he has worked to combine research, teaching, consulting and writing to build constructive links between theory and practice.

Shore has been on the faculty of Harvard University for more than two decades. He is also Distinguished Professor of Innovation and Change at Tianjin University of Finance and Economics (China) and Adjunct Professor of Organizational Development and Change at the School of Business, University of Monterrey (Mexico). Shore is a faculty member of The Governance Institute and Executive-in-Residence at Enable East (United Kingdom). He has held visiting professorships including in Amsterdam, Santiago, and Shanghai and consulted on six continents. Shore offers executive education seminars and facilitates strategic planning and guided implementation retreats. He has delivered hundreds of keynote addresses and workshops across multiple industries.

Previously, Shore spent two decades at the Harvard School of Public Health, where he served as associate dean. He founded and directed the School's flagship programs: Forces of Change; The Trust Initiative; and the Certificate Program on Launching and Leading Successful Change Initiatives.

He has served on and chaired numerous boards and committees, and currently is a founding member of McKinsey & Company's External Implementation Advisory Board which has a primary focus on operationalizing strategy for transformational change. He also sits on the Board of Directors of the Marshfield Clinic Health System, as well as several editorial boards. Among his honors is the Doctorate Fellow from the Academy of Multi-Skills (United Kingdom). Along with innovation and change, Shore's body of work focuses on creating higher performing organizations including building and rebuilding trust; reimaging and reengineering meetings; and, strategic marketing and branding.

His most recent books include Launching and Leading Successful Change Initiatives in Health Care Organizations: (Jossey-Bass, 2014); Forces of Change: New Strategies for the Evolving Health Care Marketplace (Jossey-Bass, 2012); and High Stakes: The Critical Role of Stakeholders in Health Care, with E. Kupferberg (Oxford University Press, 2011). He has also written a book on trust and edited a second.

Professor Shore has been recognized as a Top Thought Leader in Trust by Trust Across America – Trust Around the World. He is also a multiple recipient of Harvard's Letter of Commendation for Distinguished Teaching Performance, including in 2017 and 2018; and was chosen to deliver a 2015 and 2016 Harvard Lecture That Lasts (Harvard's version of a TED Talk) and a TEDx Talk.

Previously, Dr. Smith served as the founding Chief Medical and Science Officer at the West Health Institute, president of the West Health Policy Center in Washington, D.C., and the manager of the West Health Investment Fund. Prior to that, Dr. Smith was vice president of Emerging Technologies for Johnson & Johnson, senior vice president and Chief Medical Officer of Guidant/Boston Scientific’s Cardiac Rhythm Management division, and Professor of Medicine and Biomedical Engineering at Washington University in St. Louis.

Before entering leadership roles in the industry, Dr. Smith was a practicing cardiologist for almost twenty years, training at Brigham and Women’s Hospital in Boston, with subsequent sub-specialty training in clinical cardiac electrophysiology, and with faculty appointments at Washington University in St Louis and Johns Hopkins University.

An international speaker on healthcare innovation, Dr. Smith has testified before various Congressional committees on the value of healthcare technology and has served on the Food and Drug Administration Safety Innovation Act (FDASIA) workgroup. Weagraff has both a bachelor’s in nursing and a master’s in business administration. Dr. Leana Wen is the Commissioner of Health for the City of Baltimore. An emergency physician and patient and community advocate, she leads the Baltimore City Health Department (BCHD), the oldest, continuously-operating health department in the United States, formed in 1793. BCHD is an agency with a $130 million annual budget and 1,000 employees committed to improving well-being and combatting disparities through education, policy/advocacy, and direct service delivery. BCHD’s wide-ranging responsibilities include maternal and child health, youth wellness, school health, senior services, animal control, restaurant inspections, emergency preparedness, STI/HIV treatment, and acute and chronic disease prevention.

Facing an unprecedented number of people dying from opioid overdose, Dr. Wen issued a blanket prescription for the opioid antidote, naloxone, to all 620,000 residents of Baltimore. Since 2015, this program has saved over 1,900 lives. Dr. Wen has testified in front of the U.S. Senate and House on Baltimore’s efforts to combat the opioid epidemic, and in March 2016, she was invited by the White House to speak on a panel with President Obama about treating addiction as a public health crisis.

Under her direction, the Baltimore City Health Department leads the country in health innovations, including: B’More for Healthy Babies, a collective impact strategy resulting in a 38 percent reduction of infant mortality in just seven years; Vision for Baltimore, an initiative to provide glasses to every child who needs them; Safe Streets, a program to engage returning citizens and hospitals in treating gun violence as a contagious disease; and Healthy Baltimore 2020, a blueprint for health and well-being that enlists all sectors to achieve the ambitious goal of cutting disparities in half in ten years.

Before her appointment in January 2015, Dr. Wen was an attending physician and Director of Patient-Centered Care in the Department of Emergency Medicine at George Washington University (GWU). A professor of Emergency Medicine at the School of Medicine and of Health Policy at the School of Public Health, she co-directed GWU’s Residency Fellowship in Health Policy and co-led a new national collaboration on health policy and social mission with Kaiser Permanente. The author of the critically-acclaimed book When Doctors Don’t Listen: How to Avoid Misdiagnoses and Unnecessary Tests, Dr. Wen has given six popular TED and TEDMED talks on patient-centered care, public health leadership, and healthcare reform. Her TED talk on transparency in medicine has been viewed over 1.8 million times.

Dr. Wen received her medical training from Washington University School of Medicine in St. Louis and Brigham & Women’s Hospital/Massachusetts General Hospital in Boston, where she was a Clinical Fellow at Harvard Medical School. A Rhodes Scholar, she studied public policy and economic history at the University of Oxford. She has served as a consultant with the World Health Organization, Brookings Institution, and China Medical Board; an advisor to the Patient-Centered Outcomes Research Institute and the Lown Institute; and as national president of the American Medical Student Association and American Academy of Emergency Medicine-Resident & Student Association. In 2005, she was appointed by the U.S. Secretary of Health & Human Services to serve on the Council on Graduate Medical Education, an advisory commission to Congress. In 2010, she served as Chair of the Young Professionals Council, a global leadership network of medical, nursing, and public health professionals.

In addition to her extensive scholarship in public health and patient safety, Dr. Wen has conducted international health systems research in Rwanda, D.R. Congo, Nigeria, South Africa, China, Singapore, Slovenia, and Denmark. She has published over 100 articles, including in The Lancet, JAMA and Health Affairs. She is regularly featured on National Public Radio, CNN, The New York Times, The Washington Post, and USA Today.

Dr. Wen has received recognition as one of The Daily Record’s 100 Most Influential Marylanders, Maryland’s Leading Women, and Top 100 Women; Baltimore Business Journal’s “40 under 40,” The Baltimore Sun’s 25 Women to Watch, and Modern Healthcare’s 50 Most Influential Physician Executives and Leaders. A Fellow of the American Academy of Emergency Medicine and Academy of Medicine, she has been a Visiting Professor for Alpha Omega Alpha Honor Society and for Johns Hopkins Hospital. In 2016, Dr. Wen was honored to be the recipient of the American Public Health Association’s highest award for local public health work, the Milton and Ruth Roemer Award. In 2017, she was named one of Governing’s Public Officials of the Year.
