Empowering the Patient: Pfizer's Disease and Care Management Programmes for Management of Patients With Chronic Conditions
Pfizer's disease and care management programmes implemented in Finland, Italy and UK aimed at empowering people with long-term conditions. This choice was made following the many efforts to improve the quality of health care for people with long term conditions. This was a result of the growing evidence supporting the inclusion of patient empowerment as key component of care, according to the Chronic Care model. The main difference among programmes implemented in the different countries is the telephone care versus the face-to-face care management intervention. All programmes have shown good results in terms of feasibility and efficacy, as well as satisfaction of all individuals involved, including GPs, care managers and patients. Significant mean reductions have been obtained in clinical outcomes like HbA1c and blood pressure values. Very promising results have also been registered in terms of life-style changes and medication compliance. Promoting "patient empowerment and self management" has proven to be effective and should be considered as the starting point for any primary care intervention.
