Wendy Ferrell-Smith, MHI, BSN, CCM, is the Director of Value Based Performance at St. Elizabeth Physicians. She graduated nursing school from Marshall University in Huntington, WV and received her Master’s in Health Informatics from Northern Kentucky University. She is also holds a certificate in Care Management. Prior to beginning her nursing career, she served 4 years active duty in the United States Air Force.

Outside of her role at St. Elizabeth Physicians, Wendy remains active in volunteering through her church and annual American Heart Association “Heart Chase” team leader. She is also a member of the 2018 Leadership Northern Kentucky class through the Northern Kentucky Chamber of Commerce.

Samantha Fisher is the Electronic Health Records and Quality Improvement Coordinator at Community Nurse Health Center, a Federally Qualified Health Center in the Western suburbs of Chicago. In her current role, her experience includes population health, quality management and EHR technology. She has successfully navigated her organization through the NCQA PCMH process and implemented sustainable clinical processes throughout multiple sites to ensure the highest rates of success. She utilizes the EHR system to design solutions for clinical staff to allow them to spend more quality time focused on the patient while safeguarding against charting errors. She has created these clinical process solutions in an integrated environment that includes medical, dental, and behavioral health. Samantha received a graduate degree in Healthcare Administration from Alma College.

Sandra Ford has been an Integrative medicine practitioner focusing on providing Primary and Behavioral Health care services to patients in primary care clinics in rural northwest Florida for several years. Sandra enjoys as part of her medical home team, access to a Licensed Clinical Social Worker within the medical home environment. Patients are evaluated and, when appropriate, referred on for further behavioral health services using the warm-hand-off process, an essential program for treating the whole individual. Sandra is also clinical director of two rural PanCare health clinics, as well as a volunteer provider of Women’s health services at a local free clinic. She relies on effective collaboration with other health providers to address the often complex care needs of her patients.

Sandra’s expertise includes treatment and management of comorbid conditions that include chronic medical and mental health disorders, as well as management of chronic pain disorders implementing adjunctive non-opioid medical therapies. Sandra is a graduate of Valparaiso University (Indiana) holding dual Masters Degrees as both Clinical Nurse Specialist (CNS) Adult Mental Health, Nurse Educator (NEd). Additionally, she holds a Post-Graduate Certificate as a Family Nurse Practitioner (FNP-BC) and is ANCC Board Certified in that area.

Sandra has enjoyed presenting at several Medical and Behavioral health conferences; most recently, in November of 2017 for the PCMH World Congress in Orlando.

Nicole Harmon leads HANYS Solutions’ Practice Advancement Strategies. She has tremendous experience in the areas of primary and specialty care practice transformation, cross-functional teams, establishing a culture of change and quality improvement, redesigning workflow and responsibilities, and electronic medical record implementation and reporting. In addition, Ms. Harmon works on physician engagement and physician payment reform strategies to round out the overall Practice Advancement Strategies portfolio.

Ms. Harmon is a nationally recognized speaker on topics such as positioning physicians for success in the transition from fee for service to value based payment models, change management, the role of culture in transformation efforts, and practice transformation in both primary and specialty care.

Before joining HANYS Solutions, Ms. Harmon was a Practice Manager, Patient-Centered Medical Home, at The Exigence Group, and served as Assistant Director for Advanced Primary and Specialty Care at Independent Health. Ms. Harmon’s experience also includes the Windsong Radiology Group, where she spent two years as Marketing Specialist before moving into the Organizational Development Supervisor role.

Ms. Harmon holds a Bachelor of Science degree in Business Administration from Rochester Institute of Technology and a Master of Business Administration degree from Canisius College. In November 2014, Ms. Harmon completed the Cornell-HANYS Academy for Healthcare Leadership Advancement. She is PROSCI® certified in change management and by National Committee for Quality Assurance (NCQA) as a PCMH Certified Content Expert.

Lanea Jueco actively serves as the Director of Nursing at Golden Valley Health Centers, a PCMH-licensed practice. Lanea works closely with staff training to ensure PCMH integration and collaboration between departments for continued transformation. Previously, Lanea worked as the Director of Nursing at the Telecare Stanislaus Psychiatric Health Facility, and was the Manager of a psychiatric inpatient facility of Doctors Behavioral Health Center at Tenet Healthcare prior to that role.

Dr. Kuzbyt graduated from Marquette University with a major in Psychology and a Sociology minor. He earned a Master's Of Science degree in Applied Psychology from the University of Baltimore. Dr. Kuzbyt later attended the American School of Professional Psychology where he earned his Doctor of Psychology (Psy.D.) degree. As part of the program, Dr. Kuzbyt completed externships at the Washington County Hospital System and the Maryland Department of Corrections, both in Hagerstown, Maryland. He completed his APA Accredited Internship as Chief Intern at the Northeastern Ohio's College of Medicine (NEOUCOM) in Akron, Ohio.

Dr. Kuzbyt moved to Florida in 2000 and began his practice in Federally Qualified Health Centers (FQHCs). The mission of these not-for-profit organizations is to serve the indigent and uninsured people of the community. Dr. Kuzbyt initiated and developed a fully integrated behavioral health program and department at Thomas E. Langley Medical Center in Sumterville, FL. Part of this program included working with the 5th Circuit Court and the County Court systems to ensure that people with behavioral health needs would receive treatment. In the later 9+ years at Langley Medical Center, Dr. Kuzbyt began more administrative work and less direct patient care. For the last 4+ years, Dr. Kuzbyt worked at a community health center in Collier County doing similar work. In January 2013, Dr. Kuzbyt made the decision to return to seeing patients and also elected to open a private practice.

Additionally, in 2004, influenced by a significant amount of forensic work that he was doing, Dr. Kuzbyt entered law school part-time at Barry University, Orlando. He graduated in 2008 with his Juris Doctorate and was subsequently admitted to the Florida Bar. While Dr. Kuzbyt does not actively practice law, he utilizes the skills and experience from law school in many facets of his work.

Dr. Kuzbyt has presented both nationally and within the state of Florida on the benefits of integrating behavioral health practice into the primary care settings, including presenting at the 2012 American Psychological Association National Conference, in Orlando, Florida.

Shawn Martin, senior vice president, advocacy, practice advancement and policy at the AAFP, is responsible for overseeing the AAFP Division of Government Relations and the Robert Graham Center for Policy Studies in Family Medicine and Primary Care in Washington, DC, as well as the Division of Practice Advancement at the AAFP headquarters in Leawood, Kan. In this role, he directs legislative and private sector advocacy on issues such as physician payment and medical liability reform.

Prior to joining the AAFP senior management team, Martin served as director of government relations and health policy and director of socioeconomic affairs at the American Osteopathic Association. He began his career at AOA as the assistant director of congressional affairs from 1999 to 2000, when he was promoted to deputy director of government relations and director of congressional affairs. In 2006, he was named director of government relations and health policy and, in 2011, he also became director of socioeconomic affairs at the AOA.

Martin has served on the National Quality Assurance Coalition Patient Centered Medical Home Advisory Board, the Agency for Healthcare Research and Quality Practice Transformation Advisory Board, the Hospital and Facilities Accreditation Program Patient-Centered Medical Home Advisory Panel. He serves in leadership roles of several coalitions in Washington, including a six-year period as chair of the Health Coalition on Liability and Access. He also is an active member of the Patient Centered Primary Care Collaborative, where he has served in several official leadership roles since its inception in 2006.

In 2010, Martin received the Ryland Medal for Health Policy from the New York Institute of Technology, and in 2009, he received the Oklahoma Osteopathic Association President’s Citation. He was named by The Hill as one of the top 10 health care lobbyists in Washington, DC. A native of Oklahoma, Martin earned his Bachelor of Science degree in business administration and marketing from Phillips University in 1992.

Dr. Elaine J Soriano currently acts as the Regional Medical Director, Internist and Pediatrician at Golden Valley Health Centers, a PCMH-licensed practice. Elaine is an Internal Medicine Specialist in Modesto, California, having graduated with honors in 1986. With more than 31 years of diverse experiences, especially in Internal Medicine, Dr. Elaine J Soriano affiliates with many hospitals including Doctors Medical Center, Memorial Medical Center, and cooperates with other doctors and specialists in medical group Golden Valley Health Center.
