Beyond P4P: What it took to Improve Care and Reduce Costs in a Primary Care Group
Quality incentives are necessary, but insufficient. Working in partnership with a 12-physician group and Healthways, HealthSpring found a way to align data, dollars, and nursing care support within the day-to-day work-flow of Sumner Medical Group. The result was an unprecedented increase in clinical quality resulting in a significant decrease in avoidable ER visits and hospital admissions.
  1. Learn the practical realities of engaging MD groups in a P4P partnership.
  2. Learn the practical realities of creating fair, robust outcomes metrics using the medical record (not claims) as the basis for measurement and payment.
  3. Learn the practical realities of re-designing practice workflows to fully leverage physician extenders to reduce practice variability and increase quality of care...while reducing the demands on the MD’s time.
  4. Learn how health plans, physician practices and health improvement organizations can effectively integrate efforts to enhance coordination of patient care.
