| Dr. Daniel Conti serves as a First Vice President at JPMorganChase, the parent company of the second largest bank in the United States. At JPMorganChase he serves as Manager of the Employee Assistance Program. He has developed and implemented innovative programs that address the link between behavioral health status and corporate costs and has published significant research on this subject and other health economics topics. He currently serves on the Board of Directors of the Depressive and Bipolar Support Alliance (formerly known as the National Depressive and Manic Depressive Association DMDA), the Mental Health Association of Illinois, and as Adjunct Associate Professor at DePaul University in Chicago.

| For over 20 years, Dr. Heidel has provided behavioral health services in the workplace. Services for individuals include executive assessments and coaching, threat assessments, fitness-for-duty examinations, and independent medical examinations (IME). System interventions include organizational assessment, crisis management, conflict resolution, risk management, and team building. He also serves as an expert witness. Dr. Heidel founded Integrated Insights, a licensed health plan, in 1979 and served as President and C.E.O. until July 2003. He used his clinical and business skills to administer an organization providing behavioral health services at the worksite.

| Ms. Wooldridge manages the Employee Assistance Program (EAP) and the Work/Life Balance department at Delta Air Lines, Inc. In this capacity, she developed an internal EAP to deliver comprehensive services to 70,000 employees and engineered the collaboration between EAP and Work/Life initiatives to ensure integrated service delivery. The EAP has received recognition for outstanding contributions to workplace mental health including; the 2000 North Georgia Chapter of EAPA Award of Outstanding Leadership in Combating Depression and a 2002 National Health Care Purchasing Institute Award for Innovations in Employer Mental Health Programs.
