Research consistently finds that the quality of medical care that the elderly and disabled receive through the Medicare program is lower than it could be. In late 2006, Congress passed legislation that will require physicians to report on their compliance with quality measures beginning this year. The move is a prelude to having Medicare pay physicians based on the quality of care they provide. In an article just released by the Yale Journal of Health Policy, Law, and Ethics, Cato Institute director of health policy studies Michael F. Cannon argues that allowing Medicare to "pay for performance" is a sure way to ruin the concept. "Congress should confine P4P to private Medicare plans and encourage participation in those plans," Cannon said. "If Congress puts Medicare in charge of P4P, the effort will be impotent and costly at best."
