Improving the coordination of care across the continuum is a system-wide effort. Health system leaders are implementing strategies to improve transitions of care, ensuring that the right level of care, support, and case management is provided, and putting the additional resources necessary to serve patients with complex care needs. The goal: eliminating gaps in care, enhancing care delivery, reducing readmission rates, sharing data and information more effectively, and ultimately improving outcomes for patients. The Care Coordination, Transitions, & Complex Care Management Track brings together leaders from hospitals, health systems, and their partners across the continuum to share strategies and best practices to achieve those very goals.
