According to the study “The High Concentration of U.S. Health Care Expenditures” by the Agency for Healthcare Research and Quality (AHRQ), the “elderly (age 65 and over) made up around 13 percent of the US population in 2002, but they consumed 36 percent of total US personal health care expenses…A principal reason [being] a much higher proportion of the elderly than the non-elderly have expensive chronic conditions.” As the baby boomer generation ages and the numbers of senior citizens with multiple chronic conditions increases, innovative care delivery models are being developed and utilized to better serve this population, while simultaneously bending the cost curve. By attending the World Congress Leadership Summit on Innovative Care Delivery Models for the Aging Population, you’ll ensure that your organization is well prepared to attract and properly manage this growing, high-risk population.

Showcasing successful and cost effective care delivery models, this Summit will convene senior executives from hospitals, health systems, health plans, and long term care facilities to address such topics as:

  • Improving transitions between acute and community based care
  • Strategies for effectively coordinating and managing prescriptions for seniors
  • PACE, Guided Care, NICHE, GRACE and other innovative care delivery models
  • How Patient-Centered Medical Homes are serving the aging population
  • Transforming your organization into a center of excellence for senior care
  • Payment and reimbursement models driving change in hospitals
  • Best practices for reducing senior re-admissions
  • Using telehealth, remote patient monitoring, and health
  • Activating patients and their caregivers
