Many are aware of the evidence that those who are socially isolated or lonely, experience housing instability or food insecurity, or limited transportation options, have worse health outcomes than other patients, even when appropriate health care interventions are prescribed or recommended. These disparities have been particularly underscored during the current pandemic where underprivileged and vulnerable populations have been most impacted. And while the health care industry, in collaboration with its community partners, has increased its focus and made great strides on addressing the social determinants of health, many opportunities still exist to innovate, measure, and pay for services. Hear from two health care visionaries on new efforts and innovations, as well as additional opportunities the industry should consider to support whole person care, and align payments with the outcomes we hope to produce that acknowledge and service the social influences impacting health.
