The Role of HIE in Meaningful Use: Connecting to Your Ambulatory Physicians Disparate EMRs

Santa Cruz HIE has been exchanging data in a multi-stakeholder environment for more than a decade, but now with the adoption of EMRs, data needs to be to be seamlessly exchanged across the entire community � in order to achieve Meaningful Use. With over 300 providers, 42 specialties, and 100 locations, Physicians Medical Group of Santa Cruz County (PMG) is the organization that supports and connects them all to each other via the Santa Cruz HIE. As the nation�s longest running HIE, the Santa Cruz HIE enables electronic clinical data communication between PMG providers, 2 hospitals, County Health Clinics, 7 national and local reference labs, 5 imaging centers, and several safety net clinics.

In 2008 Santa Cruz HIE led the interoperability charge and connected disparate HIS and EMR systems, Clinically Networking them to enable complete interoperability to improve patient care and practice efficiencies. Using an Interoperability Hub, physicians will easily meet the clinical data exchange objective for meaningful use. For smaller practices that don't already have an EMR, Santa Cruz HIE offers an EMR Lite solution, providing immediate interoperability with the entire HIE and EMR functionality enabling practices to apply for stimulus funds - leaving no physician behind. As a result of attending this session, attendees will be able to:
  • Achieve bi-directional HIE in a true multi-stakeholder environment
  • Understand and address the challenges of connecting EMRs into a HIE
  • Perform medication reconciliation across a community
  • Manage a Community Master Patient Index

The World Congress Leadership Summit on Leveraging the Stimulus Funding to Accelerate EHR Adoption will convene representatives from hospital and health systems, medical societies and associations, states and health information exchanges, as well as medical group leadership to discuss strategies for financing an EHR purchase and implementation of technology that will meet criteria for meaningful use. A key focus will be on the stakeholder's role in supporting physician and medical group technology adoption and optimizing the use of these tools post-implementation.
