The 6th Annual Leadership Summit on Readmissions

Led by national thought leaders on transitions of care, this workshop focuses on creating new revenue streams for hospitals, health systems, and ACOs. With inpatient revenue diminishing, providers are creating new revenue streams with post-acute and transitional care programs. These not only improve the patient centered care model, but also act as stand-alone profit centers for the health system. These new revenue streams also enhance a provider’s MSPB (Medicare Spending per Beneficiary) and BPIP (Bundled Payment Initiative Program) efforts.

Specific focus areas include:

• Remote monitoring and telehealth programs
• Risk stratification
• Electronic communication with post-acute providers
• Third party ambulatory care management and effective management of home health and other home-based services (including homecare/non-clinical caretaker services)

Workshop leaders describe steps and strategy to identify critical resources and help plan and launch programs of this nature through an exploration of case studies and best practices. Participants brainstorm how one or more of these programs would fit into each other’s continuum of care model. The workshop also benefits commercial payers, dual plan managers, and physicians with details of how to operationalize tactics that increase revenue through program development, preserve the ACO (and BPIP) risk pool, and reduce readmission penalties.
